About SD United

Leveraging the work of groups like the San Diego Veterans Coalition, and the region’s Peer to Peer hub at Courage to Call, SD United is a care coordination network aimed at improved access for our Veteran/Military Families, and enhancing the rich community collaboration in our sector. Built on the fundamental pillars of collective impact, this network is being built across 13 key community service areas affecting our military and veteran community with shared outcomes to ensure that resources are best deployed to ensure no one falls through the cracks. The goal of SD United is to build and maintain with quality and efficiency a trusted Care Coordination system that results in long-lasting and impactful results and easy access for our military, veterans and their families.

SD United is not a new program, but a new way for community providers to collaborate in better serving our Veterans community.

This initiative builds on the work of groups like the San Diego Veterans Coalition and Courage to Call to give a backbone technology to provide an integrated care coordination network to serve our Veterans.

The end goal of the network is to ensure a smoother path for providers and clients to access services in the community, while tracking 100% of case outcomes to ensure no Veteran is falling through the cracks.

Using a HIPAA compliant care coordination software, providers are able to collaborate and share information across the network in real time.

This shared approach allows providers to focus on their area of expertise, while leveraging a larger network of established providers across a wide array of critical services to ensure their clients are receiving the services they need.

Under the guidance of a community based steering committee of Veterans and Military community service providers and experts that also support the Courage to Call peer-to-peer network, SD United is operated by 2-1-1 San Diego in partnership with the San Diego Veterans Coalition.
